This form is for patients actively receiving treatment for beta thalassaemia major (Cooley’s anemia) or those caring for them who need advice on welfare benefits and assisted travel (e.g. blue badge, disabled person’s freedom pass, etc) only. We will aim to respond within 5 working days.
