Make a Referral

The referral form from our website has been removed as of 2nd February 2026. If you have been set-up to refer directly to our services using a unique link, please use this going forwards.

If you have not been given a unique link to make referrals onto our system, then please complete this form and we will add your email address to our system. This will enable you to make referrals directly onto our case management system. We have a daily cap for this form, and once it has reached capacity, the link will state this, so please try again the following day.

We respond to referrals Monday to Friday (closed on public holidays), and aim to get back to the client within 7 working days.

Is there a Data Sharing Agreement with your organisation in place?
We will use the information you provide in your enquiry to set up direct referrals from your organisation into our system and send you your unique referral link. If we need to contact you regarding this, we will use the details provided to do so.
