Thalassaemia Patient Self-Referral
This form is for thalassaemia patients only who need advice on welfare benefits and assisted travel (e.g. blue badge, disabled person’s freedom pass, etc). We will aim to respond within 5 working days.
Please note that this form will only allow you to submit if the information you provide shows you are a thalassaemia patient and your problem is something this service can help with.
